SJCCC Employee Change Form	Used to initiate changes with the Human Resources/Payroll Department like change deductions, banking information, etc.
Canada Revenue Agency TD1	Used to deduct extra income tax or to take a tax credit at source versus when filing annual return.
Sun Life Extended Healthcare Claim Form	Used when submitting claim for Extended healthcare (perscription drugs, medical equipment, etc.).
Employee Assistance Program	The Employee and Family Assistance Program offers assessment, referral and treatment. Common examples addressed in counselling include marital/ individual conselling, parenting, work/personal stress management, substance abuse, grief counselling and depression. Our team consists of bilingual social workers who have Master's of Social Work Degrees. Male and female counsellors are availible. The Program can also be access by calling 1-800-663-1142.
